Privacy
Notice
When you are admitted, you will be asked to
acknowledge receipt of the hospital's privacy notice and choose whether you wish
to be listed in the hospital directory. The notice describes how medical
information about you may be used and disclosed and how you can get access to
this information.
Patient Rights and
Responsibilities:

Insurance concerns related to adverse decisions
by your managed care health insurance plan should be directed to the financial
counseling department at ext. 6878.

If you are 18 or older and mentally competent,
you have the right to make decisions about your medical treatment. If you want
to control decisions about your healthcare even if you are unable to make or
express them yourself, you will need an Advance Directive. An Advance Directive
is a set ofinstructions explaining the healthcare you want to receive in the
event you lose the ability to make decisions for yourself.

There are three kinds of Advance Directives: a
Living Will, a Healthcare Power of Attorney, and Advance Instructions for Mental
Health Treatment. You can obtain 'A Personal Decision' information packet from
your social worker or by contacting the patient advocate at ext.
4303.

A copy of your Advance Directives can be kept
on file at the hospital. You will need to bring a copy with you or ask a family
member to do so.

Ethics Committee is available to help you or
your family address such issues related to appropriateness of care and
end-of-life decisions. To contact the Ethics Committee, please call ext.
4321.

Delnor staff is committed to providing you with
the best possible healthcare. Our responsibilities to you are:

To treat you with consideration and
respect.

To assess your pain and to manage it with
you.

To ensure all communications and records
pertaining to your care are treated as confidential.

You have a right to have all of your questions
answered by the physician before you giveinformed consent for any
procedure.

To make sure you clearly understand all
hospital rules and regulations that apply to you or your
family.

To respond to your requests for service as best
we can. These services include evaluation, treatment, and when no further
services are indicated, referral to another facility in a responsible
manner.

To provide education to you or to a family
member, friend or guardian on your behalf, regarding your diagnosis, treatment,
prognosis and pain management options.

To ensure that your consent, except in a
medical emergency, includes specific information regarding procedures
and/or treatment, the medical risks involved, the probable time for recovery,
and reasonable alternative to the procedure and/or
treatment.

To respect your right to refuse care after you
have been informed of the consequences, if no legal conflict is
involved.

To explain clearly to you whenever your
participation in a special research project may be desired to obtain from
you an informed consent at that time.

To inform you of your continuing health
requirements upon discharge.

To provide you
the opportunity to receive an explanation of your bills regardless of the
source of your payment.
