Discharge
Planning
Nurses will assist you in planning for your discharge by determining the
level of care you will need.

If it is determined
that you need additional services beyond your discharge a social worker/case manager can
help you access community resources and assist with transfers to an extended
care facility or rehabilitation facility as needed.

| Social workers/Case managers can also assess continuing care needs, such as home nursing and home
therapy services. They also can help you arrange for assistance with
chores, meals, etc., and can direct you to companion services and
financial and insurance resources. |
When your primary physician(s) determines you are ready to leave the
hospital, a discharge order(s) will be written. Once your discharge order(s)
have been signed by your physician(s) it will take your nurse approximately one
hour to process your paperwork and discuss your home care instructions.
In preparing to leave, we suggest you arrange for a family
member or friend to pick you up. If you are unable to leave by your designated
discharge time, please let us know so arrangements can be made to accommodate
you until someone arrives. Your room needs to be thoroughly cleaned before
another patient can be admitted. We ask that patients leave their rooms
at the designated time in order to ensure our hospital's efficiently.
Home Care
Instructions
Your nurse will work with your physician(s)
regarding your home care instructions. She/he will spend time with you going
over the discharge instructions. If you have questions about your diet,
activities or other matters, please make certain you ask your nurse or
physician(s).
If you have any questions after your discharge
once you are at home, please contact your physician(s) office.
Medications
If your physician gives you a prescription(s),
you will need to have it filled at your local pharmacy.
Personal
Belongings
Before leaving your hospital room, please collect
all of your belongings and double-check closets and drawers. If you have
anything stored in the hospital safe, call the security office at ext. 4444 and
an officer will bring your belongings to you.
Transportation
Service
When you are ready to leave, a member of the
hospital staff will escort you to the front entrance and help you into the car.
Home Health
Services
| There are occasions when patients need additional
treatment or care after they are discharged from the hospital. To meet the
special needs of these patients, Delnor's full-service Medicare-certified home
care agency, which supplies nurses/nurse aides, medical social workers and
physical and occupational therapists for home healthcare. Private duty
nurses, nurse aides and adult sitter services for discharged patients are also
available through Home Health Services. For more information, call

Home Medical
Equipment
If you require medical equipment and/or oxygen,
we will arrange for the equipment to be delivered to your home. For more

Health Ride
After your
discharge, you may need to return to the hospital for a physician appointment,
additional medial treatment or tests. For homebound patients with no other
means of transportation, Delnor offers a free shuttle service for patients to
and from their home for such appointments. To
