What services does Delnor Home Health provide? 

Delnor Home Health offers skilled intermittent nursing, physical therapy, occupational therapy and speech therapy.  Medical social work and home health aide services are available to those patients who are receiving another skilled service (RN, PT, OT or ST).

How soon will a nurse or therapist be out to see a patient? 

This is based on medical necessity and physician orders.  Same day visits can be scheduled if needed; most other patients will be seen within 24-72 hours of referral or discharge from the hospital.

How often will the nurse or therapist come to the home? 

An individualized plan of care is developed at the initial visit which includes the frequency and duration of treatment.  This varies according to ordered services and patient needs.

Who pays for home health services? 

Delnor Home Health is licensed to provide services under Medicare and is contracted with most major insurance companies.  Please call (630) 513-0370 and talk to our intake specialist regarding your coverage for home health care.

Can Delnor Home Health provide home medical equipment? 

Delnor Home Health provides equipment for the treatment of obstructive sleep apnea (CPAP and BiPAP).

Does a patient have to be homebound to receive home health services? 

A patient must be homebound to receive home health services under Medicare Part A and most insurance plans. They describe homebound as "leaving home takes considerable and taxing effort." A person may leave home for medical treatment or short, infrequent absences for non-medical reasons, such as a trip to the barber or to attend a religious service. Delnor Home Health recently began offering therapy services under Medicare.

Part B. Under this program, a patient does not need to be homebound to receive services in the comfort and privacy of their home. To be eligible for Part B services, a patient must have a qualifying diagnosis and the potential to achieve functional goals. This plan may cover pre-operative visits, or may assist patients who are unable to schedule visits or transportation to an outpatient facility.

Can Delnor Home Health provide homemakers and home health aides? 

Delnor can only provide a home health aide for personal care if the patient also has skilled needs and is receiving another service (RN, PT, OT or ST) from our agency.

What if a patient only needs therapy services and has no nursing needs? 

Delnor Home Health is able to provide therapy services for patients with no skilled nursing needs.

Can Delnor Home Health provide lab services in the home? 

Yes, lab services can be provided in the home but they must be ordered as part of a comprehensive treatment plan that includes other skilled services.

How do I get started with Delnor Home Health Care Services? 

If you or a family member is a patient at Delnor Hospital, please ask to speak with Rita Fox, home health liaison, or a discharge planner. Skilled home health services require a physician order.  If you are at home and have a doctor's prescription, please call Delnor Home Health at (630) 513-0370 to begin the process.  If you do not have a prescription but feel that you want or need our services, please call us as well.  Our friendly staff can review your needs and contact your doctor if necessary.
