Social Services
The social services department at Delnor provides patients and their families with counseling and referral services to help cope with the problems that often accompany an illness or hinder recovery and rehabilitation. As members of your health care team, we are devoted to helping people function the best they can in their environment. You can reach the social services department by calling (630) 208-4126.
Our Staff
Delnor’s social services staff is experienced and dedicated professionals with masters degrees in social work and counseling. As members of your health care team, our staff can provide information and support as well as serve as a resource for services available in the community. Our staff can help ease the worries and concerns that can prevent patients from obtaining the maximum benefit from their medical care and can offer assistance and guidance to meet a variety of emotional and social needs such as:
- Community resources
- Placement options
- Home health/hospice options
- Transportation
- Mental Health Services
- Medication Resources
- Community resources for drug and alcohol treatment
- Emergency housing
- Grief/bereavement
- Discharge planning
- Identifying a patient's clinical picture at the time of discharge
- Providing information for the patient and family about appropriate resources
- Identifying where to get or rent home care equipment
- When necessary, making arrangements for air travel to get the patient back home, using either a medical flight or domestic flight
End of life Planning
- Living Will an explanation on how to fill paperwork out, and what they mean in a clinical setting
- Identifying care options – going home verses going into an assisted living facility, nursing home, and hospice benefits in either placement
- Informing patients and loved ones regarding comfort measures versus aggressive medical treatment
- Discussing ethical concerns
- Conducting family conferences
Maternal/newborn services interfacing between community agencies and patients, including:
- Adoption agencies
- Health services to children and families
Debriefing hospital employees during difficult cases:
- Provide voluntary service for employees
- Grief counseling for staff
- Provide help to identify when outside counseling should be considered
